Did they go and buy a second book? because the one you completed goes poof when you get a new one.

Yeah I don't know how they accidentally lost a book. The only way to get rid of one is to actually go to the NPC and choose to dispose of it which is very unlikely to be done accidentally.

You can also select to throw it away directly in the key item tab in the inventory. No need for an NPC.

Are you sure he isn't just freaked out about the first one disappearing when he purchased the second one? You can't have more than 1 book at a time so when you buy the second one the first is removed/ destroyed/ what have you. Of course, I suppose the situation might be that they bought a new book before finishing the first...can't say I've ever had cause to try that since finishing a book takes a lot less time than grinding 1500 tomes.
