You may have trouble dealing with the horizontal dives if you only rely on sound splash cues. I don't see how I can make my charts look simpler and easier to read though :/

Quick and dirty:
Imagine your boat is like this
S S
S S
S S
D = Danger, S = Safe, x = splash location
Vertical dives do this:
x
D S
D S
D S
or reversed.
Horizontal dives do this
x D D
S S
S S
Levi can always do either vertical dive. However, he may only do two out of three horizontal rows at once - only the two closest to him. After he does one dive, he will do a second one from the other end of the boat, following this rule (so if he starts top, he can do either Vertical(left), Vertical(right), Horizontal(top) or Horizontal(middle). After this dive, he can do either Vertical(left), Vertical(right), Horizontal(bottom) or Horizontal(middle)).
For slams, use a combination of these tactics;
1) watch other people - if you are the only one moving in a certain direction (or staying where you are), it may be wise to move!
2) You know it's going to be after two dives (after one around yellow spumes), so if you don't see a splash after a second or so, run towards where you're facing.
3) Sound cues, but this is obviously a problem for the hearing-impaired.
If you have "look behind you" keybound, you can use that while running to confirm whether it's behind you or not too.

It is a really well done fight. The only thing that sucks is you can be too good at it and force an auto wipe from pushing phases too fast.

I have an issue with the cheap dev tactics of using Add's in a Primal fight (not talking Splumes but Sahagin) rather than primal abilities & attacks.... Just ruins the feeling of a Primal fight.
Also the cheap tactics of "falling off".
Other than that i like the fight as well & soon to be added to DF which a bonus

Although i think its quiet similiar to Titan EX Mode (a plattform that you can fall off, avoid pbaeo attacks, avoid frontal line attacks etc.)
i would like to see some different fight mechanics with ramuh or shiva ... not necessarily avoiding mechanics... maybe something more like battle mechanics "Paralyse and DoT effects by thunder etc." avoid attacking Shiva or ramuh while they have a damage shield on them etc.. otherwise it will reflect magic and physical damage etc,.
the last battles have been more focused on evading aeos attacks, rather than battle mechanics.

Levi Ex is a great fight. Lots of fun, well balanced.
