It can be a bit confusing I guess... but it's basically based on what you've paid for (billed to your CC, drafted from Crysta Passbook, or pre-purchased with time cards), not how long you've been active. We've been active since Beta4 in August 2013, but unless you bought the prepaid time cards, you didn't actually start paying for it until October 2013. Between Beta4, Early Access, and compensated time due to access issues, we got almost 2 months free. So, at best we've paid for ~180 days even though we've been playing much longer (that is, if you've been billed through your 6th month). If you haven't been billed for April, then you may not have qualified for Rank 4 yet depending on just when you started to actually pay.
Oh, and you're talking to players about it because this is a user-to-user forum(hyperlink is to the sticky welcoming you to the forums, in case you haven't seen the guidelines that explain that). At best, you should expect an SE rep to just direct you to the details as listed elsewhere on one of their web pages, something like what is posted in the Mog Station on the matter:

Accumulated Subscription Days
Accumulated Subscription Days" refers to the total number of days subscribed to your service account, rather than actual play time.
This total will not be reset.
Selecting 30 days for your first subscription purchase will grant you the Rank 1 Veteran Rewards automatically.
Purchase of an additional 30 day subscription will bring your total up to 60 days, granting you the Rank 2 Veteran Rewards.
Furthermore, purchase of a subscription exceeding 30 days will grant Veteran Rewards based on the corresponding subscription period.
Ex: Purchasing a 90 day subscription will grant the following:
Rank 1 (30 days): Wind-up Cursor Minion, Fantasia
Rank 2 (60 days): Black Chocobo Chick, Cascadier Uniform Voucher
Rank 3 (90 days): Beady Eye, Voidal Resonator
Periods of Inactivity
In the event there is a break in your subscription, the period between the expiration and renewal of your subscription will not be included in the total.![]()
