PS4 Issues - PS3 upgrade, loging in on PS4, linking PSN, code not working...

I bought the PS3 version on disc and decided to upgrade to PS4 version.

I upgraded and it gave me a code, I used in the PS Store and it comes up with FFXIV Standard Edition, go on continue = This code has been used.
I tried PS4 store, PS3 store and internet store and same thing every time.

I downloaded beta phase 1, played and made a character.
Downloaded beta phase 2 and DID NOT play.
When I go on my beta phase 2 now it sais I need to purchase the digital download or use a promotion code. (which doesn't work)

On the mog station is sais PS4 standard edition - unactive.

Then it turns out my PSN has unlinked itself from my SE account.
I try to link and it sais it has failed because my PSN account may be incomplete.

Also my PS3 version no longer works too.

I also tried changing password, email, no difference.

My PSN is the same as my PS3 one too,

Please and help would be HIGHLYappreciated.

On a PC:
1) Go to http://www.sonyentertainmentnetwork.com and log in to your account
2) Select Redeem Prepaid Card and enter your code

On your PlayStation®4:
- Select [Account Management] -> [Redeem Codes]

That should help with downloading the game to your playstation 4 console.

I can't help you with linking your sony network account to your Square-Enix account, I am afraid for that you need to contact support...

I try same : The Prepaid Card has already been consumed.

I wnat ask you I can use Final Fantasy XIV: A Realm Reborn (US) UGC-$15 for subscription ? or now ?
