just got the physical game with a 30-day subscription code, where/how do i use it?

I got my hard copy of the game today too, Square Enix said that for people who bought the hard copy and need to register the code are having issues putting it in the ps4 store, I tried mine and it says my code is invalid. so until this issue is resolved I don't think you can enter the code until SE resolves this problem.

oh ok, i also used the pre-order code for the cait sith and moogle hat but its not in my inventory, where do i find it?

You should get them in in-game mail. Find a moogle to pick it up.

I would suspect you use the 30-day code (and game registration code) at Mogstation rather than Sony's site?
