Thank you for posting this, as you've pretty much outlined the exact problem (less the Beta Phase 1) that I've been experiencing.

Following is my current "troubleshooting" history of this issue:

I've contacted SE Customer Support, and they say they cannot provide assistance with PSN issues that may be related to promo codes (though maybe they just didn't look deeper into the issue). So they referred me to Sony, which I contacted. They say they have the promo code on record and that it has been redeemed, with no further uses available. After I explained that it didn't work, they referred me to Amazon to try and resolve it. Amazon mentioned they'd send me another promo code once the physical product has shipped.

So right now I'm waiting for that to happen. If that doesn't work, Sony has offered to escalate the issue and possibly attempt a fix with the promo code in their system records. Also, SE suggests that if I needed additional assistance with my order, I should contact Digital River directly via phone at 1-866-585-3251. I'm not sure if this relates only to if you placed the order through their online store, or in general.

Let me know if you find a solution in the meantime as well. This has been quite a hassle to get early access to the game. :-(
