    The general assumption is 5-10 minutes a FATE, then add in travel time between them. I use ~15 minutes as an average between the two.

    Technically video games are programmed in hexadecimal usually in increments of 256, so 0-100 is actually 0-255 (or 0-FF if you want specifics.) Technically it IS impossible for an item to never drop unless the rate is set to 0. It is entirely possible for an individual player to never see a drop.

    Statistically speaking you need very large samples to accurately determine drop rates unless we're actually told what it is. If 100 people report in on a drop and one person received it, then you might assume a 1% drop rate with 1/100. If another 100 report in with 0 drops then the % drops to 1/200, or 0.5%. Another 100 report in and 3 people got the drop then it becomes 4/300, or 1.333%. It's entirely possible for some players to always return a 0 on their drop rates even with others still getting theirs. It's why some people are still farming the Deathcharger on WoW and some players have been religiously running Molten Core since Vanilla and don't have Thunderfury. A completely random drop with such a low drop rate is just that: completely random. Some players have an Atma, some have 2, and some are working on #3. And then several players are still farming their Atma or even trying to just get their first one.

    Because not everyone begged for it. Typically players don't post much when they're content and tend to assume a "silent majority" that everyone is actually with them. Either their needs/wants are met and most stop posting, or their needs are not met and they quit the game and stop posting.
