Its like dodgeball...if the ball went at your direction and you evaded it, then its evade...if the dude was able to toss the tall 3 feet away from you, then he missed...basically the points is who did the action...the monster missed or you evaded.
OK if it was dodgeball in FFXI (range attack) sure if you miss or evade either way you wont be able to counter because you have some distance in between the mob and you.

I'm talking about close range combat; when the mob is right by you. I wrote this specifically in Puglist section of this forum since this thread topic is for Puglist's counter moves thus being close range combat. Has nothing to do with long range.

When the mob makes his/her/it's move to attack physically the mob can't stand still and attack he/she/it has to move towards the player to hit the player OR swing their weapon/parts at you on the spot.

If the mob being a humanoid monster; it will probably step in (closer to the player) and swinging weapon (or just swing at the spot), if the mob is like dodo again if you think realistically dodo has to step towards you and swing its beak to stab you (again unless it wants to stand still and swing)... now either the mob misses or you evade, the mob moved in to attack and because of the miss/evasion it breaks his/her/it's stance and that should give you a chance for counter strike.
