To clarify: Does the Amazon PS4 CE also include the Fat Chocobo mount, even though it's not listed on the page? Or is that only available when ordering from the Square Enix store?

It is for all CE accounts.
If you buy a CE, the game code in the box would be a CE registry code with all the ingame items a CE give you
