You don't have to wait for shroud. All heals generate enmity based on the amount healed, whether it's overheal or not. The reason you don't use regen at the start of a pull is that when the tank grabs something with say shield lob, he gets aggro from the other mobs too, but only at 0 enmity. One tic of regen is plenty to change their minds. Another example is similar, except you put regen on yourself, since you haven't done anything to the mobs, OR the people who have hit the mobs yet, you don't get any enmity (unless they are showing up on your enmity bar).
