Vorpal Thrust to Full Thrust Combo action

I'm not getting the combo proc when I go through my rotation and get to Full Thrust. Anyone else having this problem?

was working for me tonight when I was spamming FATE's in Dragonhead... is it consistently not proc'ing, or does it feel more random, as in perhaps a WS whiffed?
