Game crashes to desktop whenever I am in either of the new dungeons. Brayflox HM crashes ridiculously fast during the 2nd boss fight. Halatali HM crashes during the final boss fight after the first wave is dead. I've yet to do Amdapoor City but I'll beat a Dunesfolk's sweet arse that it will crash there too! It must be a patch error as well as the Duty Finder's errors colliding.

I have the one-time password added in a week before the 2.2 patch (in case you needed that information)
Update: it crashed again while I was accessing my retainers in U'ldah (even when I try to re-login)
Update: (3/31/2014 12:35AM +8GMT) I removed the one-time password. Game still crashes

I was away for most of the weekend but I had another crash on Friday night and this time my PC really didn't like it and I had trouble booting after that.
I ended up having to format my hard drive and reinstall Windows. I got the game reinstalled last night and I'll be putting in a support ticket though the game. Thanks for the replies everyone.

Yesterday I ran The Tam-Tara Deepcroft and Copperbell Mines on an alt and everything was perfectly fine in both instances until the moment we engaged the final boss. Both times right at the last boss I began crashing to desktop and every time I would get back in the game I would crash again until after the boss was defeated. I crashed immediately upon engagement with approximately five to six crashes on each boss and, again, it did not stop until the boss was defeated. I had absolutely no issues the previous day running anything but I did not run The Tam-Tara Deepcroft or Copperbell Mines so I don't know if it's just those dungeons.
Have to hard system reset after the game randomly locks up and locks my entire PC. Audio continues playing in background (including twitch streams or even skype calls, I can even talk to people in skype while it's locked up). Nothing whatsoever in event logs other than the event 41 kernel that's caused by me hard resetting. Complete mystery. All I can tell is that it seems to happen more often on a fresh boot. I'm following some advice from folks over here: http://forum.square-enix.com/ffxiv/t...sh-BSOD-issues which includes removing display drivers from old graphics cards and going back to older, stable nvidia drivers. Not sure what the results are yet but hope it can help someone. (Note: I have a new issue now though...in-game audio cutting out when I try to hand in quests. :/)
I've had it happen everywhere from central shroud running around doing hunting log to doing fates to HM titan to dungeon instances. Just recently I crashed on Stone Vigil twice in a row (two different instances) just before the last boss. I'm pretty sure it's just awful luck, but I've crashed just before or during the final boss all week...Yet I've done Odin fates on max settings with hundreds of people particle spamming and I have YET to crash during that. I am completely boggled, I have no way to reliably replicate it.
Having the same issue. Except in my case (and my wife's case) our display driver stops responding (nVidia GeForce GTX 470). We both have identical systems. I've tried everything from drivers prior to 2.2 to the latest greatest beta. Ever since 2.2 I can tell when it is about to happen, because the framerate gets pretty choppy then the screen goes black and after a moment comes back with an error that the display driver crashed but has recovered. The game will keep running thankfully. Temps are normal, nothing in the logs. FFXIV is the only game that does it and it was flawless prior to 2.2. I chalk it up to some junk that made it into the code with the PS4 release.

Let me add +1 to at least most of the posts.
I played at launch on pc with graphics all the way up, and never had any issues.
I didn't play for a few months in which 2.2 was implemented. Since I came back after that:
Playing in Full Screen causes almost instant pc crash. Borderless window is ok until...
Now, almost anytime I have a cutscene, or change areas, or sometimes at a final boss, game crashes the whole pc. Sometimes I get the blue screen of death, sometimes just restarts pc.
Took me two days to complete one copperbell hm run because I would crash at start of last boss and get kicked from party.
Today, I'm trying to do a simple Sahagin quest that I've done a few times already, but every time the short cutscene kicks in, crash.
Again, I never had any issues before 2.2, so I don't believe anything is wrong with my rig, graphics card or graphics being all the way up.
I wanted to add a few details in case it helps anyone who is hopefully working to fix this:
I don't crash in the same places all the time, it seems to change from session to session. I've run Copperbell Hard many, many times without issue until the other couple of days, when I began to lag or crash near the end, and would flat out crash before the last fight if I got that far. I was able to run other things and generally had no problems aside from the CM HM run.
Today, I'm trying to do the Sahagin Tribe quest where you have to ride the Elbst, and I can't get past the cutscene without a crash. I've done the same thing the last few days without any issue, but now it's impossible to complete the quest.
The only common element I can see is either a cutscene, or changing areas such as teleporting or moving to a new floor in a dungeon. I hope that helps, because I love this game, but it's becoming Unplayable.
