I think there should not be limit to commendations, I don't see parties forcing to commend everyone as it's only a vanity reward... Maybe allow us to have unlimited commends on Roulette runs...If you could give them to everyone in the group you'd have runs typically ending with people insisting that everyone be commended whether or not they deserve it. I usually watch the dpsers to see if either is doing a particularly good job since tanks (and healers to a lesser extent, but that's what I play) tend to get them by default unless they really screw up.
I never had someone asked for commendation since the start of it until now, I don't think people care that much for the reward, but it is nice on the other hand to reward all players that I think should be rewarded and not have to pick one.
I come from PSO2, where there instead of a commendation you give someone a "good job" which give them exchanables for rewards (and even valuable ones, not just vanity) and still I have never came across a player who asked for it, or cared for it or parties that been stating "commend only"...
Same from me. I try to be as friendly and as talkative (within reason) as I can be, but when someone does that and then ends it all off with a request for commendations I tend to think it was all just an act. Pretty well guarantees that someone else will be getting mine.
As nice as the idea is in theory, I don't particularly like that SE assigned unique rewards to the Commendation system. It would have been better if everyone was required to give one before leaving an instance, because in my experiences, most people in PUGs/roulettes are seemingly in such a rush that they don't even bother. I've also heard (firsthand or secondhand) that some players spammed easy guildhests as soon as the system was introduced in order to hit 500 and get all the rewards, and now purposefully withhold giving commendations out in order to make their gilded mount more rare.
I love that SE assigned unique rewards for commendations. If they didn't, I would never use the DF and just enter with full parties all the time. It's also causing a change in attitude amongst players. Someone who wants a commendation is much less likely to lash out at someone who made a mistake, or is "doing it wrong." This is not a bad thing at all. If someone wants to fake nice to get something, let them.
And a big fat NO to everyone being required to give one. There have been a few parties (for me) where no one deserved one. I refuse to reward bad behavior.
And the people who spammed guildhests to get the rewards know they got it the easy way. When I finally get to 500, it will be legitimate, and I'm gonna feel really good about it.

This is it. Seriously. Be tank or healer and don't be a dick and you will get commendations fast.

Well I use the same logic, and by your post I'm assuming you do too. So that makes three of us.
Following the structure of tank <-> healer and DPS <-> DPS, yes. If one person performed poorly or someone performed exceptionally, I'll make an exception. The DPS shouldn't be protecting the healer in the first place; that should be the tank's job - for instance, if I'm healing and one of the DPS is constantly taking MoBs off of me to keep me alive, then yes, I'll give them my commendation.
