Steps:
We were a party farming Ifrit Extreme for multiple runs. On one of the runs, after defeating all the nails in the third nail phase, Ifrit used Hellfire as expected, but instead of using his last set of plumes, the full set of 13 infernal nails spawned again. This issue was only seen once over the course of 5-6 tries. Our party wiped partway through killing the extra nails, so we did not see if his Hellfire mechanic would be repeated again as well or not. After wiping, we continued in the same instance and defeated Ifrit without the issue reoccurring.
