It's not a phantom or memory. They are indeed still alive somehow. I think it's just a standoff between the two right now. 2.2 will hopefully illuminate further.

Well Nael van Darnus was absorbed by Dalamud when he was defeated in 1.23. Coil is within the remnants of Dalamud so it was only inevitable.

Not very many people have cleared turn 5 yet. Probably about 5-10% of the games population have actually cleared it.
