Afaik, offensive names are already against TOS, so I think it will suffice report the player, that after the fun of making such names, will have then the fun of a sanctioned account.

It's not like the names are illegal or hurtful, they're just purposefully stupid from people who jokingly just put in whatever they thought of and now that they want to be serious players demand that they're allowed to change names.
Some examples that come to mind in nothing but a 2 minute search scrolling down the list starting at A...
Akkey Spreadbeaver (Brynhildr)
Acer Scumbag (Malboro)
Ada Cucumber (Tiamat)
Ashanty Cumalot (Lich)
Alicia Ironboobs (Faerie)
Adumb Dwarfthing (Brynhildr)
Stupid Cockwaffle (Mateus)
Leaky Loli (Sargatanas)
Seriously, even though there have to be some LEGITIMATE people who dont like their names. I can easily asure you that the majority are idiots who now regret their original name choices and dont want to start over. They dont deserve it.
Such a forgiving community

I'm all for a PAID name change service, with a waiting period between name changes. The cost and length of the wait are up to SE....

Maybe a once a year item for paid service but can't pay for 2 years till the next year is closer or something not sure how that would work.
We need a setting to auto use Aetheryte Tickets ~ An amount set by the players like set to 500 Gil if a teleport cost over that amount it will auto-use a ticket!
Ahh, I want this so badly! I've been wanting to use my Fantasia to turn my Miqo'te main into an Elezen, but I've been holding off in wait for some name changing service to be implemented. I'm a roleplayer, so I'd like my name to be lore-abiding and I don't want people looking at me funny because I'm an Elezen with a tribe name.
If it was a paid service offered in the Mogstation, I would gladly shell out money for it.
If it's any comfort, perhaps there could be a trail, or an annotation when you hover over the name in your friend/block list that says something along the lines of, "Previously known as ____."
As long as the blocked list doesn't remove people when they've changed their name, I don't see an issue? And so what if the players with stupid, gross names want to change them to something nicer? If they change them, then there will be less stupid, gross names running around, thus less of a problem.

Not releasing Namingway Cards isn't going to do anything to stop a dedicated troll, there's already a trick that lets you use server transfers to change your name. If a troll isn't dedicated enough to spend the $16/$32 necessary to do so they're probably not going to feel it's worthwhile to spend the $10 or whatever it'd cost to change their name either.

I'll throw my support in for this

why should we do a name change? I mean, you picked that name in the beginning knowing full well you can't change it.
If you want a new name, then make another character. The way I see it, your character is like your child; would you change the name of your child because you don't like it anymore?
Welp, in the case of the guy on my server who's name is "Stupid Idiot" ....it's your own fault. XD Should have thought about a name you'd like to stick with in the long run.
I love my name personally. ;D (I know, you asked.)
Rex because that's a name I've always liked,
and Xylon, meaning "Wanderer of the Woods" or "From the Forest" (as far as Greek naming goes) or "One living in (such) a relation to wood or trees" (as far as the literally Greek word Xylon goes) I chose that back in 2010 because Gridania is my starting city-state. Found it fitting because in the 1.0 opening you're found just wandering in the woods alone.
I actually just found this and the name is more fitting than I originally realized! XD
Xylon: "People with this name have a deep inner desire for a stable, loving family or community, and a need to work with others and to be appreciated.
People with this name tend to be passionate, compassionate, intuitive, romantic, and to have magnetic personalities. They are usually humanitarian, broadminded and generous, and tend to follow professions where they can serve humanity. Because they are so affectionate and giving, they may be imposed on. They are romantic and easily fall in love, but may be easily hurt and are sometimes quick-tempered."
Moral of the story: Research a name you can really stand by.
