Phial of Fantasia bug / Used and no modification option

Full details: I went to use my second Phial of Fantasia, about 2 weeks after my first, and after using it, it disappearing from my inventory, gave me no on screen warning like it had before, and when I logged out it gave me no option for modification.

Now comes the headache and a half I've had with support, but this isn't a complaint for support. Long story short: automated response about non-refundable in-game currency.

I've put in another ticket about the poor support I've received and talk to a live person about the issue. Here's the bug post however.

In order for me to submit your report, I will need some more information. You can go Here and you can find templates for Windows and PS3 that represent the kind of details we need. Once you can provide such information we can move forward and resolve this issue. For now I am going to move this thread into Insufficient Information. You will not be able to reply, so once you have gathered the requested data please create a new bug report thread.
