"Bad" instance servers causing PC game to crash?

Has anyone else run into this problem, (described below)?

I join a party finder group looking to kill Garuda extreme. The group fills up to 8 and the Garuda extreme fight is initiated via the Duty Finder. At this point everything is fine, there are no problems what so ever. Everyone buffs up and is good to go. Then the SECOND the tank pulls, the entire game crashes.

I try to log back in, but while the party is still in that same instance, I instantly crash again on log in. The only way for me to log back in is to wait for the party to back out of that instance, and only then can I log back in without crashing.

After I log back in, the same party initiates the Garuda extreme fight again via Duty Finder. This time we connect to a different instance server. The tank pulls, and there are no problems at all with the game. We win, get loot and exit the instance. The same party queues up again for Garuda extreme via Duty Finder. The tank pulls Garuda and all is fine.

It is completely random, the instance server we connect to. However when we connect to a "bad" one, my game will crash every time.
