As is, I am now incapable of completing Titan HM (which I've cleared many times previously) because of frame rate drops and graphical lag causing me to eat mechanics which were previously easy to dodge. Titan EX isn't even an option.

This issue is making the game border-line unplayable for me, and I can't seem to find a cause for it on my side. I've stopped all unnecessary processes, clean-sweeped the hell out of my unused installs.. I'm at wits end on this.

Drivers are up to date? Try uninstalling/reinstalling your GPU drivers (upgrading to the most recent version). Also keep any eye on your GPU allocation. See if XIV is using 100% of it. If it's not, the drivers are not right and not allocating what it needs. If it is, check your temps and heat sinks (also could be a driver issue as well).
