How does auto-attack initiated (strange fact)

Hi guys I was wondering how can a player initiate the auto-attack? I have seen the video many time and I conclude this:
the auto-attack initiated if a player is in active mode and locked on the mob. "noticed that the normal attack is not in the actions bar".

If that's true then it gonna suck. Because it doesn't give you control over your auto-attack especially with not so good targeting system.

I'm not sure if this way is the best way. what if I don't locked on the target? what if I unlocked the mob after initiating the auto-attack, will the auto-attack stop?

is there any possibility to have the normal attack on the actions bar wherein pressing it once would initiate the auto-attack
