This problem is actually related to how drops in the coil work paired with the fact that you can only ever form one third of the group needed to enter.

Randomly generated loot with a once a week lockout means that when people want a specific item they no longer run the content for the content's sake but plow through it until they get the piece they want. Their only concern is efficiency.

Paired with the fact that you could potentially be running this dungeon with 23 strangers most people who fall into the above category stop seeing their compatriots as people but rather as faceless npcs who either know what they are doing or don't.

All wrapped up in a game in which the only forms of communication directly clash with actually playing the game. Basically this game goes out of its way to distill the absolute worst aspects of humanity and then forces you to deal with it for about an hour at a time.
