The same thing has happened to me multiple times as well (not after that many withdraws though), and is quite annoying and frustrating and a waste of my time.
as far as i know thats a totaly different issue a bug in the duty finder. I've seen that happen after 1 queue and NO one withdraw in a 4 man.
