Steps:
I passed "onion gauntlets"
1.Game put them in my armory and locked me out ( but log does not say they were given to me).
2.I checked log and this is what it said "[18:17]Unable to obtain the pair of onion gauntlets."
3.This was first loot item in party and first time in Crystal Towers after reset. I did not have these prior either.

This happened to me too, passed on blm boots and they went in inventory, didn't say I rolled or anything, quite irritating since the suit I was waiting to get dropped on last boss...

Adding my 2 cents to this as well. Last night I had very bad latency problems in CT> my receive was well over 30k multiple times when I checked it. On the last boss (Acheron) my screen froze for the last 2mins of the fight and I could do nothing. When it came back I was in time to see the last person leaving the instance and the DRG body armor falling into my inventory even though I was trying desperately to pass on it as soon as I saw what dropped. I have no interest in gearing my DRG!

After ticketing the GM and receiving a standard we cannot rectify mistakes in rolling (I didn't even get a chance to!) I would suggest that you look again at the loot system- perhaps if people don't roll either need or greed on an item it should automatically drop to no-one after a set period of time NOT go into anyone's inventory that has not passed who is still inside the instance.
