Steps:
1. Plug in PS4 controller to Windows 7 (64-bit) PC. Controller is recognized, drivers installed. FFXIV recognizes it. Great!
2. Notice that the buttons are mapped all wrong.
3. Open the config settings and press the Calibrate button.
4. Get the first prompt, to press left analog stick up.
5. Press left analog stick up.
6. Nothing happens, as if you hadn't pressed any buttons at all.
Now, before anybody points this out, I am already aware that you can edit the FFXIV.cfg file to get the buttons mapped properly. However, I am also reasonably certain that editing game files is against the ToS, so I am reluctant to do so. Currently I am using a 3rd-party program that causes the PS4 controller to be emulated as an XBOX360 controller, but to my mind this situation is also not ideal, since the game does recognize the controller, so there is no reason why it should not be able to be calibrated properly from within the game itself.
I also apologize if this has already been reported, but I did use the search function with a few different terms and didn't come up with anything.
