Steps:

I change to a save equipment set, and get the yellow exclamation mark that means something is missing. The last time was my level 50 miner whose HQ level 50 head *Hamlet Digger's Helmet*. The previous occasion was my fisher who lost her chest piece, and a normal "battle" chest piece had been equipped in its place, with the original gathering item nowhere to be found. I reported this in game, except for yesterday's discovery involving the miner.
