can try changing drivers, reinstalling, uninstall something that was recently installed and could conflict, even formatting. of course it can also depend on the game code but in that case it's "hard" to fix since not everyone is reporting this.
I've updated the drivers for the video card and I've done everything under the sun for this issue. My next step is to purge my video card drivers completely in safe mode, then re-install them. After that, I'll try install FFXIV on a different drive (60Gig SSD) and see if it is in fact a HD issue. Wish there was a solution for this, almost wondering if Microsoft security essentials is the issue.
