Did anyone make any attempt to explain where people were failing when it happened? Or did people just throw up their hands and say "this isn't going to work I'm done" and then leave duty?
The latter is what I saw more often than not while doing Garuda HM. New people would be in the event and fail someplace and sometimes people would tell them where they screwed up but often not. So the wipe happened again at or around the same point. But want to know the shocking part? When those people were told what was happening and why the party was wiping it stopped happening. They may fail elsewhere as they learned the mechanics but they learned.
Want to know what happened then? One person decided they were done, despite progress being made so the whole party fell apart.
Twelve hours is a lot of time to spend on one fight, but without context I can't help but think it's because the people in those groups didn't want to put in the effort to explain what happened and why it happened.
To the comment from AldricBranchais, what does it matter if it is progression or not? This is a game, and people shouldn't have to do homework before doing a fight. Ask if someone's new to the encounter before you start (something I don't see happening very often unless someone outs themselves) and then give a brief rundown if you feel its important enough they know going into it. Then touch on spots with problems as you go.
Not everyone learns by reading a guide or watching a video after all. Sometimes they have to burn themselves a couple of times to really get it.

Saves time, really. Also I mentioned progression because before then, there were little to no guides released on endgame (unless you did 1.0 and had prior knowledge of Garuda and Ifrit). You were FORCED to learn by doing. Now? Nope. You're more than capable of finding this easily accessible information before jumping into an encounter completely blind. This also saves your booty from the obligatory Scumbag Steve who starts moaning the instant someone has to take a minute or two explaining a mechanic. Even if you learn by doing (I can't really relate, since I learn by observing), it's really helpful for you to know what exactly you're getting into... before you get into it. Since you literally SEE the mechanic before it happens.

Evil is Evil. Lesser, greater, middling… Makes no difference.
I really don't understand what's so complicated to understand.
It says "unjustified use" is punishable.
When you votekick someone, you have to tick one of the following reasons:
AFK
Offline
Harassment
Cheating
Therefore, if you use the kick feature without one of these justification, you're abusing it.
If you believe the guy is trying to ruin your game, then it's harassment, and you're entiteled to kick him.
If not, then you are the one griefing him.
As for guides, maybe some people like to do homework before playing, or enjoy youtube more than the actual video game (why are they paying a sub, then?), but there are those who enjoy discovering the game while playing, and don't want to know everything that's coming before they actually got to play. Or just find homework boring, and are paying their sub to have fun instead of being offline researching.
Yes, some players made guides and videos.
No it's not compulsory to watch them before playing.
The first guys who beat a fight didn't have any guides at their disposal. Just because someone is not a "wold first-er" doesn't mean his own game experience should not be as enjoyable as theirs.

Typical blaming on people for daing on Titan HM. The entire introduction with the Relic quest and HM Titans is poorly made, with almost no information what you are going to expect! Not to mention that Ifrit and Garuda can be passed with normal 50 gear! New players including me, we are all dump founded once we get to titan, all of a sudden the difficulty is up not to mention MAIN reason people daing on Titan HM are the stupid servers of the game or the lagg in it! Ive seen people getting out of the plumps secs befor they are finished and after that they died !

How did they vote out the tank without your vote? I thought it had to be unanimous?
Sorry if that had already been addressed.
Majority vote. Four man parties. Player being voted out has no say. 3 voters. Majority is 2.
Since it's only 3 voters, you can't make it require 100% votes either because you'll have the reverse situation where two griefers will duo queue and hit no every time. There's no way to prevent abuse other than through bans.
Just wanted to add my personal story:
This is my first MMO, I went from knowing nothing in June to knowing quite a lot now and I'm not a bad SMN, now doing end-game stuff. But now I want to learn tanking, so I'm leveling a GLD that is lvl 22. But… the thing is, I'm so afraid of other people saying rude things to me that I don't dare to enter through DF. The same is true about my WHM, lvl 34. I only play these classes when people in my FC have time to run low-level dungeons with me. The irony in the situation is that both tanks and healers are needed in the DF :P
I don't want to level through FATEs or so since I think doing dungeons is the best way to learn your role. But I don't dare enter through DF since I don't want to be hurt by mean comments or getting kicked or something if I can't hold aggro or if someone dies while I heal or so. I'm sure I'm not the only one feeling this way.
