Being both a Tank and a Monk, I usually right click to get into a fighting stance before a fight to show i'm ready to go. Then I leveled a bard to 50, and right clicked before the pull and auto attacked the boss.

Needless to say, I never did that again XD

Like my fiance who thought hydra was a solo fight (we told him it was an 8 man) and he went in as a scholar... 10 seconds later he comes out dead and was stunned LOL he did not talk for the rest of the night

Cleric stance and not having eos out up until halfway through ultima hm fight. Was wondering why I was healing like a nub, then I realized it. /facepalm

On the last boss in AK. Had him selected and forgot....sung foe requiem and pulled boss early. Annnnnd wipe. He does NOT like to be sung at. >.<

Took me 2 hours to learn how Ifrit's Searing Wind works.. the uncountable wipes I caused for everyone.

As someone who abuses Cleric Stance... this one has been the instigator of some hilarious(ly awesome) moments for me. Not always resulting in wipes! Also, sometimes my brain will just... get switched around or something, and I'll keep trying to turn Cleric Stance off to heal someone and then back on to continue dpsing, but I end up getting stuck and doing it in the reverse order - turning Cleric Stance on to heal, turning it off to dps. xD

Yup guilty of that too. xD

I too am guilty of forgetting to pass Rot (and think this one derp threw me out of Rot rotation when we finally succeeded lol) was just too into healing that time, didn't even see it get passed to me.

Haven't sprinted into a fight yet as a tank (Sprint is on a different bar, thankfully) but I do throw random buffs on at random times since my WAR and PLD have different bar set-up...
