Same here, FIOS customer, lag has been so horrible lately. It makes me and my wife look like noobs on Twintania. Same problem, run a ip trace and it chokes up at LEVEL3. Been using the WTFast program for a couple days and it really cleared it up but its only free for 30 days. I dont feel i should have to pay $5-$10 for a program just to connect efficiently to a game im already paying $15 a month to play.
No extra programs will help. People in my FC with actual lag problems get help from Pingzapper and such, but this new problem for FIOS is not related to us. It's on SE's side. We can't do anything but complain and hope they acknowledge us at some point. Bad thing about having the best internet = you're a minority and easily overlooked. So much for being hardcore.
It's getting depressing. People log in for the weekends, needing help with Titan EX and relying on certain others, but those certain others are the ones who randomly freeze mid-fight. This lag is the worst lag in the history of FFXIV, too, because it can't be predicted. There is no way to tell when it's going to hit, but you can be sure that when it does, you'll be knocked out of the arena before landslides even begin casting. Most of my FC is just sitting in town right now, too afraid to do anything.

WTFast works perfectly (for me) actually. Since you connect via their servers you are getting a different route to the Montreal DC, presumably (since its better) not via Level3.No extra programs will help. This is how I am atm, Doing very little in game activities.
I am at titan hm with my summoner, trying to do it with my friend and a group of our friends helping us this time around. Oh lawd.... its a hit or miss game going on most of the time because we can beat the heart and keep up for a bit.... (over half gone) but lol.... when that lag hits it just all goes down the drain. xD
Edit: I guess I should add that I use FiOs as well and I am a pc player so it's not because of any ps3 lag, hehe. soooo yeah.... connection problems here and there. :x

Our FC is in the same boat. We have a few people with FIOS in the Baltimore area, all of them are having the same issue. Where we could face roll Titan hard mode before, we are lucky to finish now. You can completely forget about Titan EM. VPN services do help however I refuse to pay more for a game I am already subbing while also paying an arm and a leg for a 75/35 internet connection.No extra programs will help. Everyone keeps pointing the finger at FIOS. I've been on the phone with them a couple times now with higher level support and they are being very insistent that they are not throttling the game at all. They are sending me a new modem that we hope will help as I just upgraded to quantum.
Wasn't really pointing the finger at FIOS. Other than this game I really have had no issues and actually love the service. I'm simply saying that there is an obvious connection with this issue and FIOS customers at the moment. I'm also a verizon fios user over in Texas and today i had to leave a Titan Extreme fight due to lag so bad i couldn't even move in time for the landslide. I called Verizon and they said they couldn't do anything on their end. I also never had any issues until after Christmas.
Also a FiOS customer here, and I can vouch for the annoying connection problems that started a little after 2.1 hit.
For the most part it seems like an overall increase in latency, but I'll occasionally have huge lag spikes and lose a whole 3 seconds or so at a time too. Nothing like that ever happened before.
