Cycle through your offensive and defensive cooldowns for an overall increase in threat and survivablity. I use them one at a time and by the time I use the last one the first one is off cooldown again.

If you're a GLD use flash after each attack rotation.

Mark the main attack target #1 and once you hit it with a couple attack rotations and feel confident that you have threat on him, start tab targeting through the other mobs and do an attack combo on each one followed by a flash.
