Sometimes when I have Adlo up, I still get take damage, any reason why? As far as the tool tip goes, it doesnt specify if its physical or magical damage it blocks. Am I missing something here?

You are just lagging. The next attack will be absorbed instead. It usually happens when you heal the target a fraction of sec just before getting hit, the shield and heal did not get through fast enough.
Yes, very annoying for Death Sentences and the occassional succor...

It's animation "lag". If an enemy begins an animation (spells, special attacks, even just winding up an auto attack) before you get the buff, it won't absorb any damage.
WHM used to be able to repro this effect with Stoneskin on the last boss of WP, pre-2.1; if you didn't have Stoneskin on early enough in Everyone's Grudge, you'd get no mitigation - the skill would bypass the bubble effect.
I've actually been noticing an icon bug with Stoneskin. I'll put it on a tank, he'll do a pull, get the usual damage absorbed, then he'll start taking damage even though the stoneskin buff icon is still on him. I don't know what causes it, I don't know how to reproduce it, it just happens from time to time. I've seen it happen on dps and myself as well, so it's not just a tank thing. I'm thinking it has something to do with gaining or losing a different buff icon at the same time that Stoneskin is supposed to go away from taking damage.
Maybe the same thing can happen with Adlo? That's an awfully short window to have something like what I described with stoneskin to happen though, so I'm going to assume it's the late apply against an action that's already in motion. Some abilities just do that though, Death Sentence on Twintania being a prime example, you need to have buffs up before the cast starts or they won't count towards the mitigation.
