Since the patch on 12.24.2013. I am an avid player with a monstrous machine and great connection. Never lagged before the patch.
Frequency:
Always | Although this is the option, I can go at MOST 10 minutes without a connection drop, which last 2 - 7 seconds then everything catches up.

The entire game including Duty Finder.

Solo or Party. Anything in Duty Finder and in the world. Full Party of 8 or even 24 Alliance.
NPC Name:
Only NPC that has not responded was in Revenants Toll - Independent Mender (Mor Dhona 22,6), clearly from a spike in connection.
Monster Name:
My favorite experience thus far is getting hit off the edge in (The Navel Extreme) by a landslide that did not appear on my screen.
In-Game Time:
Since the patch on 12.24.2013.
Steps:
The steps are simple, I log in and BAM lag. It comes in waves and sometimes is less frequent then others; however most of the time its consistent once a minute for an average of 4 second lag spikes where nothing moves and then everything teleports around or catches up.
Additional information:
With my in home network I stream television and movies using Hulu, Crunchyroll, Netflix, ect. and experience no lag or connection issues at all. This was tested all day yesterdat (12.25.2013).
More specifically to connection problems, I have played Battlefield 4 at MAX settings and maintained 60+ FPS with no lag what so ever. This goes for other smaller games as well (King of Fighters, Street Fighter IV, Skull Girls) all played through my PC with no noticeable lag.
I have contacted Verizon and they report no issues with my connection on their end; however my conversation with them did not last long as I trusted their judgement before reading these forums.
The steps I have done to attempt and remedy the situation will be listed below:
Restart Computer
Turn off all additional programs
Restart Modem/Router
DNS Flush
Play FFXIV at lower resolutions and graphic settings
COMPLETE RE-INSTALL OF FFXIV (what a head ache, for no solution)
Final information:
With my computer and internet connection I have never experienced lag until this patch was implemented. I am not one to come to the forums or submit bug reports as the mass of players normally will come screaming and yelling. But this is absolutely unacceptable. Content that I was able to complete last week has become absurdly difficult and a game of chance.
