Graphic Tearing ever since 2.1

Pre-2.1 I played this game with no issues on maximum settings, v-sync off, ~45-65FPS almost all the time. Upon logging in this evening, I have this graphic tearing that is just annoying enough to want to rage.

I have tried:
Checking windows updates
Checking video card driver updates
Turning on V-sync
Turning down graphics lower
Loaded other games to see if maybe my hardware is going out.

I can alt-tab out of FFXIV(windowed borderless) and it doesn't tear, but as soon as I tab back into it, it starts tearing again. Nothing else is affected at all.

It is not a heat issue, as my video card and other hardware are running at the same temps as always.

My specs are:
Intel Core i7 980x gulftown
12gb RAM
2559MB Nvidia GeForce GTX 570

Everything is up to date and is exactly how it was pre-2.1 (no updates since then).

Does anyone have any idea what is going on here? Please advise.
