I was playing CNJ last night and on several occasions I wouldn't get any, so I can say firsthand that it doesn't go to the healer by default.
I hadn't really played CNJ much until last night and I did notice that as adjusted my macros and ended up being able to do more that I was getting commendations a bit more regularly. It does seem a fair number of people are giving commendations based on how well they play.
In the few parties that were talkative, I tended to get at least one commendation. I'd guess being friendly in chat's an obvious way to get remembered for a commendation.
Who did I give commendations to? About half the time the tank, if they were decent or friendly in chat (so many silent DPS out there). Other times if the tank was acting like a DPS or was always rushing ahead of the team too much I'd give it to the DPS that kept it together more than anyone.
Granted, I was working on leveling CNJ (now 20), so this is purely my observations from very low level stuff. Take from it what you will.
My First 'Low Level' Duty Roulette, I got 3, then I got 2 in my Guildhest Roulette, and then 1 from another Low Level DR. Yes, I was Tanking it upTwice I gave Commendation to Healer, and once to another using Duty Roulette (L50 Bard doing low level Coperbell lol)

Why *not* vote? Sure, I suppose if everyone in the team was mean and nasty, but otherwise by not going ahead and voting (since it is free) it makes you out to almost be the one that's mean and nasty. I'm not asking this because I assume you do not vote, but just as a general question to those those that consider this point.
There was one time I accidentally didn't vote, and it was then that I found out it was optional. I collected my reward while still considering who to give it to and quickly found myself out of the instance without getting a chance to vote. I suppose it should have been obvious that it worked that way, but it was late and I wasn't thinking much about it.
If you queue up with someone, you cannot vote (period). I didn't vote for one instance that was a near failure due to incompetence on the parts of the three others (all 'supposedly' experienced players who had other jobs at 50, who were utter failures as DPS and Healer)....Why *not* vote? If you queued in a partial party you cannot vote. Only people who queue solo can vote. A pop-up Notification will appear along with the Loot Notification saying 'Player Commendation'. Vote from there.

How do you vote for people? Noticed I got one after CT and was embarrassed that I just left when there was some awesome healers. Does it automatically pop up? Because I entered with a friend so maybe that locked me out from voting or something.
500 is not that bad. I've received 5 commendations in two low-level roulette dungeons as a tank and three from one DF CT as WHM. It will just take a while to work towards.
You can only vote if you went in by yourself. When you finish the dungeon, a pop up will appear that gives you the option to vote for a character or cancel it out.
