Anyone else have this problem on LNC? DEX is my highest stat, I run all accuracy accessories and I grab any accuracy and DEX I can get on gear.

My typical fight goes like this:
Invigorate
Miss
Miss
Miss
Hit once
Miss
Use TP from one hit to ferocity feint (because I'll never hit again)
Miss
Pull aggro because I turned on Life Surge (Why? I don't get this.)
