Depends how many healers are left, how much mp they have, how many other people need to be healed at the moment, what cooldowns are available, how good your gear is etc...

How could this happen?
Halone combo and Circle are very efficient against a single target, especially if you fold in fight or flight and the obvious shield oath. If a healer pulled hate on Titan for any reason other than a tank not doing combos... yes, I would blame the healer.

Is your DPS building meter in phase one forcing your white mage to cap their threat early on healing through phase 1 tumults?

Is your DPS forcing Titan to geocrush right after tumults without giving your healer a chance to get a single aoe heal in?

Are people eating near-to-full geocrush damage?

Are people standing in weights?

All reasons why white mages cap threat in titan.

Running low on mp? Learn to do your job properly!
Gets aggro and maybe dies = you're healing too much!
After a wipe= your heals are too slow
