My opening rotation is BB combo>HS(Bloodbath)>Maim(Vengeance)>SE(Unchained)>HS(Berserk)>SS(Internal Release)>BB>BB combo>SE combo (and then goes into the normal BB>BB>SE). The initial BB combo is for generating initial aggro as well as the first 2 Wrath you need for the SE + Vengeance to finish off your 5. Using the buffs in that order lets you stack them for maximal effect (they're all multiplicative and Vengeance is affected by all of them) without wasting their secondary effects (Berserk and Vengeance generate Wrath stacks that are often wasted) or losing benefits to Berserk's Pacification (Unchained and IR fall off right before Berserk). The integrated order of the attacks and buffs gives you strong initial aggro so that no one pulls off of you while you're using the SE combo and 2 fully buffed (Unchained + Berserk + IR + Maim + SE) Butcher's Blocks.
Using this, I've gotten enmity cushions so large that even my PLD OT has only a barely noticeable sliver. Those 900 damage BB crits are *insane* for enmity generation.
Keep in mind, the start of a fight is almost always a pretty safe time frame which is why you can get away with using it. Once you've gotten into the middle of a fight, you generally want to hold on to your Wrath stacks or use them on IB (since survivability is more important than damage). You should only use Unchained in situations where you know that incoming damage is low enough that you won't need those Wrath stacks or IB. The only times I really use Unchained outside of the start of a fight are on Titan during the heart phase and t2 when I'm about to take over from the other tank (Unchained cranks my damage high enough that I don't need provoke to pull; since I'm not tanking at the moment, it also gives me time to generate Wrath and the boss doesn't deal large amounts of damage until multiple debuff stacks are active). In most every other end game that isn't simply a joke to tank, the damage is too high to let the tank get away with Unchained.
