Personally I open with Infuriate and pop it immediately to build more damage enmity, but I've only been tanking WAR for 3-4 weeks now.
I always use it once my Berserk/Internal Release combo drops so I can keep the damage rolling, it's worth using it in conjunction with Bloodbath for the healing. Berserk/Internal Release>Pacification>Un-chanined coupled with your regular rotations for Emnity and keeping Maim and Storm's up results in some serious self healing.

I use it at the start of boss fights since I usually have a few wrath left over from trash mobs. Good time to use it since you're less likely to take burst damage at the start and all that. Also pair it with berserk, internal release, maim buff, SE debuff and you'll be hitting some high numbers.
Personally I start Bloodbath a good 7-8seconds before hand but then I combo 1.)Bloodbath(now 22-ish sec), 2.)Internal Release, 3.) Berserk 4.)Maim, 5.) Unchained.
The damage return from the hp of the buffed Bloodbath by Berserk and Maim and Crits from Internal Release. Unless I'm tanking Titan, healers rarely have to heal me during this time. As always (until patch time) use infuriate after popping unchained to keep your healing bonus.
My opening rotation is BB combo>HS(Bloodbath)>Maim(Vengeance)>SE(Unchained)>HS(Berserk)>SS(Internal Release)>BB>BB combo>SE combo (and then goes into the normal BB>BB>SE). The initial BB combo is for generating initial aggro as well as the first 2 Wrath you need for the SE + Vengeance to finish off your 5. Using the buffs in that order lets you stack them for maximal effect (they're all multiplicative and Vengeance is affected by all of them) without wasting their secondary effects (Berserk and Vengeance generate Wrath stacks that are often wasted) or losing benefits to Berserk's Pacification (Unchained and IR fall off right before Berserk). The integrated order of the attacks and buffs gives you strong initial aggro so that no one pulls off of you while you're using the SE combo and 2 fully buffed (Unchained + Berserk + IR + Maim + SE) Butcher's Blocks.
Using this, I've gotten enmity cushions so large that even my PLD OT has only a barely noticeable sliver. Those 900 damage BB crits are *insane* for enmity generation.
Keep in mind, the start of a fight is almost always a pretty safe time frame which is why you can get away with using it. Once you've gotten into the middle of a fight, you generally want to hold on to your Wrath stacks or use them on IB (since survivability is more important than damage). You should only use Unchained in situations where you know that incoming damage is low enough that you won't need those Wrath stacks or IB. The only times I really use Unchained outside of the start of a fight are on Titan during the heart phase and t2 when I'm about to take over from the other tank (Unchained cranks my damage high enough that I don't need provoke to pull; since I'm not tanking at the moment, it also gives me time to generate Wrath and the boss doesn't deal large amounts of damage until multiple debuff stacks are active). In most every other end game that isn't simply a joke to tank, the damage is too high to let the tank get away with Unchained.
I have a slightly different combo that maximize affect of Unchained, Storm Eye, Berserk, Internal Release Up time with 2x BB.
BB Combo> Heavy Swing> Maim> Vengence> Storm Eye > Heavy > Unchained> Berserk> Internal Release> Skull Sunder > Butchers > BB Combo again.
Internal release+Storm Eye will just wear off after your second BB after you pop them.
Anytime. I usually throw in Berserk and Inner Release for some massive burst dmg on the dreadnaguhts in T4. I'm hitting him 900+ in crits and I'm no where near close to a full ilvl 90 set. I can only imagine the damage when I'm full ilvl 90. Here's my macro:
Unchained Berserk
/micon "Unchained"
/ac "Unchained" <me>
/ac "Berserk" <me>
/ac "Inner Release" <me>
The reason why they are in that order is that it takes roughly 3-4 seconds after Unchained has been activated (assuming you're mashing the buttons) to have both Berserk and Inner Release up, leaving you with that much time after Unchained has ended to throw in a Fracture to keep the damage going while you're pacified.
