Unable to obtain item for "Wonder and Lightning" Fate

Here's a repost, since I don't have the item in a retainer and had already posted that dropping the quest and restarting it didn't fix the issue.

Time: For the past several days, various times PST
Frequency: Always
World name: Excalibur
Character name: Azalin D'nari
NPC name: Lightning
Monster name: Clockwork Dreadnought
Class/Level: Paladin/45, SMN/50
Party or solo: solo/party/solo with chocobo
In-game time: 7:44pm
Area and coordinates: Coerthas 30X 8Y
Steps:
1. Accidentally deleted the original item
2. Attempted to redo the Lightning Fates multiple times
3. Get the unable to receive item notification
4. Abandoning and restarting the quest did not resolve the issue.
