Happens randomly to me too. In instances only, though. 90% of the time I don't have this issue but it comes and goes as it pleases.... On some days I can't run instanced content at all :/

sometimes its your wifi. try pinging your router and check to see if the spikes when ur lagging.
I think it is most likely to be at neither your end or the server, but somewhere in the middle (if your connection is otherwise OK, and works in other games without issue). Routing to the SE data centre seems very hit and miss, with lots of ISPs having issues, I suffered from this terribly and I'm on a 70meg fibre connection. In my case Battleping using East Coast proxy completely fixes the issue (I'm playing from the UK)... worth trying out the free trial, but your results may vary.
I never lag but yesterday I was having some strange problems with UI menus taking ages to pop up in dungeons (WP device windows) and my spells kept getting interrupted for seemingly no reason.
Its not ur end, nor server (well some server heavy load). We get bottlenecked at somepoint on our way to the server. As Shaone above says Battleping fixes this problem. And I myself will most likely get a subscription for a service (VPN) like Battleping. They route you a faster and different way to the server, where they eliminate many of the internetts bottlenecks.
But this only makes me MAD.
Im playing from Europe, Norway. And there are none EU servers.
So my suggestion to SE are these 2.
1. SE make their own VPN (Battleping) service for EU players so they get the fastes way to the server.
or
2. Reduce EU players monthly cost subscription with a few bucks. So we can use the saved money on subscribing services like Battleping. (yes they cost everything from 2 dollar to 8 dollar a month)
If none of the above is done by SE, all EU players pay for a game they cant play without horrible lag at peek times.

I think it's an instance server congestion issue.
While I'm a vocal advocate of the 0.3sec positioning check delay fixes, this is not that. This is lag and is likely caused by your internet connection. The positioning check delay is something different entirely.
I used to get this due to my wireless router.
This comment cannot just be thrown around as an absolute.
When the game started i experienced the same thing the OP and many other people describe when talking about lag (The attack for 10 seconds, everyone else stops for 5 seconds, then zooms back to normal, rinse/repeat). I was playing wireless as i didn't have much alternative at the time. Decided to get a second modem so i could play hardwired. That completely, and entirely stopped the problem i had consistently for the first two weeks of the game. I've been playing virtually lag free.
While the game surely doesn't handle poor connections well, it's not correct to tell someone "oh well thats just the way it is until SE does something". My own personal experience shows me that it was in fact on my end. So while that may not be the case for most people, there surely are people out there who are in the same boat i was in that have options to fix connection issues without going to extravagant lengths.
