OT dropping so fast means he's not dodging pox. That's a problem.

Oddly enough, my group didn't find consistent success in T4 until our SCH started focusing on me as MT while the WHM focused on the OT and party. You think that wouldn't be the case, but it is for us...
Whoever said that OT shouldn't be tanking every trash mob in Phase 4 is not true. He should be tanking everything and he should be dodging Pox from the Rooks by running.
If you are not killing the Phase 3 Dreadnaught within 10 seconds into Phase 4, you guys have a DPS problem (most likely your dps not dpsing optimally). Try using food and potions or make it mandatory for your DPS to use. Your group's equipment is not bad at all, but a few ilvl 90s should help.

Off tank can go into sword stance while DPSing the dreadnaught, ideally you wanna get rid of phase 2's soldiers and knights at the same time the bugs drop for feeding, then kill the dreadnaught before the rook/spider phase drops. If it takes too long for your dps to engage the dreadnaught then try to improve your dps on the soldiers and knights first.
Use potions if you have to and save your cooldowns for it, that's the single hardest phase in the whole fight.

Have the scholar dps for phase 3, the whm + fairy should be enough to heal through the damage.
