You may need to get in touch with support. You might want to try email support, though I'm not sure if you'd be forced into phone support or not.

I am just curious, why not use the smartphone token software? You get access to an emergency password if something happens to where your phone is stolen, dead and so on, you can remove the token and maintain access to your account. Physical token doesn't get this option and I have heard that it has been taking several weeks for SE to assist with removing those tokens.
