Dunno about bulges, but in the sense of butts they actually reduced it on everything simply to make it easier to model gear ontop of (I.E reduce clipping), instead of using a parametric mesh deformer which is a shame.

I've not seen any nipples that are skin colour though

tested in the character creator, in terms of visibility
Highlander > Midlander (due to muscle slider) > Elezen=Miqo'te

Lalafellas and Roegadyns don't appear to have any but that's another story

Highlanders are definitely the most sexualized male characters aside Roegadyns. Like female Miqi'tties and female highlander/roegadyns.
