I have a razer with a 3x4 pattern on the side like yours. I stacked quickbars 1-4 on top of each other then created 3 grids of 3-columns x 4-rows with a space in between each. The left grid is alt + the 12 buttons, the center is just the 12 buttons, and the right is shift + the 12 buttons.

My other 2 buttons aren't on the top like yours, they are just left of the left click button. I mapped them to a couple situational things. I don't use them in battle.
