Well an exploit by definition is using an "unintended game mechanic" that's not reported and is abused by players to progress themselves. Doesn't have to be a hack. The Demon Wall skip was an exploit. People found out about it and abused the shit out of it. Another example below.
Using WoW as an example: back when Ice Crown was in full swing (Final raid in the Wrath expansion) people were able to use cheap goblin bombs to easily kill the last boss. They did not report it and continued to abuse it. Hence it was labeled an exploit. They were all banned for it and was fixed in a later hotfix.
