While it happens that the tank fail, this is most likely true.

I did very few duty finder Garuda HM, but most fails were due to either healer using aoe heal on add pop, or people not coming back to the middle fast enough for last phase.

It can help to ask the offtank to save provoke to compensate bad timed aoe heal, but if the healer keep using it after provoke, you're screwed.

In Duty Finder, WHM should be saving Shroud for the sisters so that the offtank can easily establish hate. WHM should NEVER throw regen on a tank when sisters are about to pop -- they will be blinking red before the offtank can even touch the red sister (if the red sister spawns far away from the group). I'm not saying it's your healer's fault... but it is usually a combination of players causing an issue to happen. There's a lot of give and take with enmity in this fight. DPS should also be careful not to get hit by unnecessary AoEs. (Both sisters will AoE, as well as Garuda herself.) The more dps get hit = the more the healer has to AoE spam = the more hate the healer is going to pull from the tank. As always, dps should be careful not to attack something BEFORE the tank/offtank pick it up just in case they pull hate and require extra healing.

Shroud is great for that moment when they spawn. A lot of time when they spawn even if you have nothing up they will aggro on you as the healer anyway it just takes people picking them up and you to pop shroud so they both go green then you have a lot of wiggle room to heal. The damage thing also helps if the DPS don't drag their add on top of the healer like some groups like to do. I'll normally sit in the back pocket between tornados and prefer the dps to hold the add slightly higher up so i'm just out of range of her AoE and the ranged dps can also pile in with the healers so it saves a lot of damage to heal. Some groups unfortunately drag the add into that hole and force AoE spam just to keep the run alive. Melee should sit slightly higher and get aggro on the add so it can be nuked it safety with just melee needing heals.

Even had an off tank who started dragging their add into that hole and when everyone yelled at them they insisted they were doing it right and we were wrong. The healers and ranged don't want to anywhere near Garuda it's far too dangerous XD. The top right pocket opposite Garuda is always best to only have the off Tank there until the first add dies then they can move it down for a fast nuke. Most over healing is caused through panic heals through the inexperienced healers seeing three enemies in a closed space doing damage or by group spike damage from bad add placement or dps positioning.

I have been grinding Garuda, and I have been tanking the red one on NW corner with MT, should I move the red one to NE corner? will healer be able to reach far NE corner?

If the rest of the group is stacked in the NW corner and you tank in the NE, the healers should have no problem reaching you.

@OP: Sounds like bad luck, Ifrit and Garuda are both beaten in DF very regularly. Only Titan is the roadblock that DF usually doesn't overcome. It happens, but rarely.

What I do as an OT in this fight is to keep using Heavy Swing when I know Garuda is about to spawn adds so when the adds come down, I can use Skull Sunder on the closer add. If it's red, great, combo in a Butcher's Block and pull it away from party. If it's green, stop hitting that add and let dps take aggro. Also, make sure you don't stand between main-tank and Garuda so you don't eat Slip streams. This part and the double tornado part is what make or break a Garuda run. Everything else is easy.
