    Sadly, this is usually the case. It's either an extremely horrible tank or a healer (namely WHM) who likes to spam the crap out of AoE heals (Medica 2).

    While it happens that the tank fail, this is most likely true.

    I did very few duty finder Garuda HM, but most fails were due to either healer using aoe heal on add pop, or people not coming back to the middle fast enough for last phase.

    It can help to ask the offtank to save provoke to compensate bad timed aoe heal, but if the healer keep using it after provoke, you're screwed.
