Hotbar Ability Replaced by Meld Materia Action Randomly

The ability on hotbar 1 in slot 10 (key 0) is replaced with the Meld Materia action, regardless of what ability was there, and regardless of what class I am on.

Time: 2:18 pm roughly Pacific time.
Frequency: Occasionally
World name: Hyperion
Character name: Arkain Stormfury
Race/Clan: Hyur Highlander Male
NPC name: Various. Most recently the Maelstrom Personnel Officer, but also Merchant & Menders in Costa Del Sol and Mor Dhona.
Monster name: <none>
Class/Level: Most recently Dragoon lvl 50, also has happened on Archer and Marauder.
Party or solo: Has happened both in small parties of 2-3 and solo. Most recent was solo.
In-game time: Don't remember
Area and coordinates: Limsa Lominsa Upper Decks
Steps:
1. Interact with an NPC, such as turn in items for GC seals, or make repairs.
2. Exit interaction menus.
