Steps:
Taking path killing nodes in Sectors I, II, and IV, which leaves abilities Firestream, Chain Lightning, and Allagan Rot on ADS. At ~60% after ADS used Allagan Rot, ADS proceeded to use High Voltage 3-4 times consecutively, first silenced by BRD, second silenced by PLD, and third going off. (4th High voltage was used shortly after, however it seemed to be part of the normal ability rotation)

No limit break was used prior to this occurring so as to skip phases, etc.
