Duty Finder Party glitch

Was registering for Dzaemal Darkhold with my FC on Shiva when one of my members disconnected. When he was in the process of disconnecting the commence option came up and 3 of us accepted but the timer ran out and instance was lost due to missing party member. When said party member returned I had withdrawn to try anew. This is where things started to get a bit... unique. First I was not allowed to register my party for a duty. Secondly I switched over leadership to the party member who disconnected. We were able to register but at least one member was not accepting however we all had accepted to commence. I myself logged out and back into the game after that did not work and then the weirdest moment of all. I was allowed to register party for Darkhold but the 3/4 glitch came up again. This time however the disconnect party from said instance timer did not work. I was unable to withdraw while other friends had a timer up to 23 minutes. Very rough party glitch that I would like an answer to or a repair please.

Area and coordinates: Urth's Gift, Duty Finder
Steps:
1.Withdrew from first attempt at Dzaemal Darkhold with only three members and 1 minute left after losing in boss fight.
2.Picked up fourth member and restarted duty finder.
3.4th party member disconnected and duty finder did not commence instance due to missing member as is protocol.
4.Missing member logs back in and I, the leader, restarts duty finder.
5.In attempt to restart duty finder I cannot choose an instance and start.
6. Switch leadership over to disconnected party member.
7. All commence but duty finder only shows 3 members have accepted.
8. As protocol of duty finder, instance does not start.
9.I log back out and in.
10. I regain leadership of party and start duty finder again.
11. Same only 3 members have accepted glitch occurs.
12. Timer does not start instance and retries to establish new instance.
13. I withdraw from instance again.
14.I can not start any duty again.
15. Log out and in.
16. Start duty and commence with 3 man only glitch.
17. No option to withdraw and no timer to cancel.
18. One party member states that timer says 23 minutes as mine says only 4 minutes.
19. Everyone gets frustrated and logs out except one member who is still playing.
